ПРОГНОЗИРОВАНИЕ РИСКА ПОРТАЛЬНОЙ ГИПЕРТЕНЗИИ У ПАЦИЕНТОВ С ЦИРРОЗОМ ПЕЧЕНИ В ИСХОДЕ ХРОНИЧЕСКОГО ГЕПАТИТА С

Аннотация

Введение. Развитие синдрома портальной гипертензии у пациентов с ВГС ассоциированным циррозом печени – ключевой момент естественного течения заболевания. Несмотря на приемлемые диагностические характеристики эластографии сдвиговой волны (shear wave elastography) в отношении диагностики цирроза печени, ее роль в диагностике портальной гипертензии требует дальнейшего уточнения.
Цель исследования – разработать и оценить модель прогнозирования риска портальной гипертензии у пациентов с циррозом (ВГС) печени.
Материалы и методы. В исследование были включены 388 пациентов. Оценка риска портальной гипертензии осуществлялась на основании моделей логистической регрессии. Для моделей рассчитывались отношения шансов (ОШ) и их 95% доверительные интервалы. Внешняя валидация моделей осуществлялась на выборке тестирования.
Результаты. Разработаны модели логистической регрессии, включающие скорость сдвиговой волны в печени (модель №1, κ = 0,106, AUC 56,0% (95% ДИ 47,8-64,3), или скорость сдвиговой волны, длину селезенки, уровень тромбоцитов, возраст пациента (модель №2, κ = 0,478, AUC 78,8% (95% ДИ 68,5-89,2)).
Заключение. Использование дополнительных клинико-лабораторных показателей совместно со скоростью сдвиговой волны в печени в составе модели логистической регрессии позволяет повысить качество прогнозирования риска портальной гипертензии у пациентов с циррозом печени.
