БАКТЕРИАЛЬНАЯ ТРАНСЛОКАЦИЯ В ПАТОГЕНЕЗЕ ОСЛОЖНЕНИЙ ЦИРРОЗА ПЕЧЕНИ
Аннотация
Введение. Понимание физиологии взаимодействия бактерий кишки и организма хозяина, особенностей бактериальной транслокации на начальных стадиях и в случае прогрессирующего ЦП подчеркивает важность подходов, минимизирующих миграцию микроорганизмов и их компонентов из просвета кишки. Цель исследования – краткий обзор публикаций, освещающих проблему бактериальной кишечной транслокации – основного механизма развития бактериальных инфекций и провоспалительного статуса при циррозе печени. Материал и методы. Выполнялись изучение и анализ англо- и русскоязычных статей с глубиной поиска 30 лет, содержащихся в базах данных PubMed, Cochrane Collaboration, UpToDate. Ключевыми словами были: «транслокация кишечной микрофлоры», «бактериальная транслокация», «маркеры транслокации». Результаты. Рассмотрены современные представления об изменении кишечного барьера, систем врожденного и адаптивного иммунитета при заболеваниях печени. Приведены данные о возможности и значимости обнаружения бактериальной транслокации. Обсуждаются современные методы, используемые для анализа микробиома кишечника, а также некоторые области для будущих исследований. Заключение. Для изучения бактериальной транслокации при циррозе печени необходимы валидированные маркер/маркеры.Литература
